Home Visitation Services

A home visitation program is available to pregnant women and families with young children. Services include parenting and prenatal education, growth and development assessment, and information/referral regarding community resources. Services may be requested by community providers with client consent or by individuals.

All services are voluntary, confidential and free of charge.

To make a referral, use the attached form and fax it the Brown County Public Health Division at 920-448-6449 or call 920-448-6400 and press #4 to speak to the intake nurse.
