We Offer:
  • Information & Assistance - staff is available by phone and by appointment in our office or your home. We listen to you and your unique situation, focusing on identifying needs, preferences, and support-both personal and financial. Staff will explore and link you to programs and potential funding sources so that you can make decisions that are right for you.
  • Long Term Care Options Counseling - staff provide one on one consultation to help persons understand their choices when making decisions about where to live, what kind of help they need and where to find it. If you or someone you know are considering a move to an assisted living facitiy, staff will help think about planning care, how to pay for it, and how long personal resources might last.
  • Functional-Financial Screen - A comprehensive assessment to uncover needs and determine eligibility for funding sources that help pay for long-term care services.

Located in the atrium of the Aging & Disability Resource Center, 300 S. Adams Street, Green Bay, WI. The resource room has many books, video's and brochures for you to browse through or check out.

Information by topic at your finger tips. Select from the list on the left and on the Forms & Documents and Links pages for valuable information, such as, lists of service providers, booklets, flyers and websites.

Community Services Database-211 is an online resource database developed and maintained through a partnership between the Aging & Disability Resource Center and Brown County United Way. It allows you to search for services to get more information. Searches are based on key words or on programs if you know the name of the organization or agency. It's available 24 hours a day on any computer linked to the internet. Anyone can receive the same 24/7 support via phone by dialing 2-1-1.
