How To Apply for Services

Online Application
You may apply for BadgerCare Plus, Medicaid and/or FoodShare online by using the ACCESS website. The website is: You will need to click on the box that states Apply for Benefits.

You may apply for all services by contacting the Bay Lake Consortium at 1-888-794-5747.

Face to Face Application
You may apply for all services by coming into the agency and making an in-person request. This can be done Monday through Friday between 8:00 and 4:30. If you are only requesting the health care programs, you may pick up an application and return it to the agency. If you are requesting health care and/or other programs (FoodShare, Child Care), you will need to complete some brief paperwork and will then meet with client registration staff who will set up an appointment for you with an eligibility worker.

Mailed Application
You may request a application forms be mailed out to you by calling the Bay Lake Consortium at 1-888-794-5747. You can complete the applications and send them back to the agency. An appointment will then be set up with you, if one is needed.
