Obtaining health care benefits is among the top priorities for our veterans. Please be aware that ALL veterans will have to fill out a 10-10EZ form to be seen at a VA clinic. You must fill out the financial portion - You may fill out the form by following this link, or call our office to have it mailed to you. We can also assist you with any questions and review your form with you after you have finished filling it out. (Please make an appointment with a specialist in our office for this service - 920 448 4450.)

Transportation to and from appointments is made possible by the Disabled American Veterans Transportation Program. Please call (920) 431-2641 at least two weeks in advance of your appointment. Please ask for their current rules and restrictions if you are not already aware of them, or visit our
transportation page.
